At Elevate Home Health, and as a member of our team, we will work together to help build Elevate into a strong company providing excellent home healthcare to residents across the the Regions 4 and 7. As a member of our clinical team, we know your clinical expertise will help ELEVATE our outcomes and footprint within the area.
We are actively recruiting for LPNs with the desire to serve patients with compassion and personalized care in their homes-whether that be personal residence, long-term care or assisted living.
Our team of nurses provide clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
Requirements:
Minimum of one year nursing experience in the acute care setting with previous home health experience strongly preferred.
Current and unrestricted LPN licensure
Current CPR certification
Strong organizational and communication skills
Valid driver’s license, auto insurance and reliable transportation.
